List each of the other team members. For each, give three numeric evaluations:

Their contribution to the product
Their contribution to quality efforts
Their contribution to team process

Use a scale from 1-5 for each:

Very little contribution in this area
Minimal contribution in this area
Meets expectations, but generally did not go above and beyond what was requested
Strong contribution in this area
Outstanding contribution in this area, often making up for incomplete work of others

Note that all 5’s doesn’t make sense! Fractional scores are acceptable, but half points rarely make a big difference.

In addition, write a sentence or two explaining each score. You can format your information using a table if convenient. Note: this does not translate directly to a grade! We are well aware that different people have different perspectives on a project. Our primary goal is to ensure our understanding of how students are performing aligns with what has been observed by team members. The reports will be treated confidentially; they will not be shared with teammates.
